Colette Boroch is a certified Chronic Care Professional and Health coach. She has been working in the Nephrology field of medicine for 10 years. She has always gone out of her way to make patients feel comfortable, no matter how uncomfortable it could be being at the doctor’s office – talking about kidney disease, and getting blood drawn may be! Looking at the “whole person” when care planning is something that has helped her express absolute understanding with her patients. Helping people find their way, coaching them in ways to better themselves and overall improve their quality of life is what drives compassion and empathy within her work. She finds fulfillment in empowering patients to become activated through their own hands in their care plan and supporting those who need help getting back on track. She believes that setting goals in life to better ourselves is imperative for overall wellness – and speaking on goals forges motivation to ultimately achieve.
Andrea DeMarco is a seasoned health and wellness expert with over 11 years of experience in empowering individuals to achieve sustainable health and wellness. She is a Board Certified Health and Wellness Coach, Functional Nutrition and Lifestyle Practitioner, Nutritional Therapy Practitioner, and Lifestyle Medicine Health Coach, with advanced training in Integrative Nutrition Health Coaching.
Andrea specializes in intensive lifestyle interventions, focusing on personalized weight loss strategies, GLP-1 management, and overall wellness optimization. Her holistic approach integrates nutrition, behavior change, and lifestyle modifications to help clients create lasting transformation. With a passion for educating and supporting clients on their health journeys, Andrea combines clinical expertise with a compassionate, individualized approach to wellness.
Afrina Khan is a certified holistic health coach. Through her 1-on-1 and group coaching programs, she has supported her clients in making step-by-step changes to achieve a healthy diet and lifestyle. Whether it’s weight loss, increasing energy levels, managing health conditions, and managing stress, she’s worked with clients through health education, giving them the resources, support, guidance and the accountability they need to live a healthier life. Now as a care manager, she is using those skills to help her patients take better care of their health and improve their disease outcomes .
Kyla Hansen is BLS Certified through the American Heart Association as well as Certified Medical Assistant Phlebotomist via the United States Marine Corp. With over seven years of clinical experience, she has made the transition to Chronic Care Manager. Connecting with her patients on several levels to help them feel most comfortable and ensuring that their needs and goals are being met is something she prides herself on. Encouragement, compassion and empathy are three core values she has shown to organically deliver to each patient. As a child she was diagnosed with Meningeal Encephalitis, resulting in a coma. She experienced nothing but overwhelming amounts of love and guidance from the nurses, doctors and staff upon waking whom all displayed such selfless acts of kindness for the better of a child and their family. This experience has influenced her to take extra time with patients in need and to truly value a dedicated care team to providing optimal support.
Over the course of her career, Gabriella Arrigo has held a variety of jobs in the patient care field. From paramedic, to patient care technician, to medical assistant, and now to Care Manager. Patient care is her number one priority and she always strives to go above and beyond for her patients to make sure those who are in need are taken care of. The diverse patient care served between emergency, bedside, and administrative has given her the needed experience to navigate patients through to achieving optimal care goals. She is currently working towards her bachelor’s in Clinical Psychology. When she is not at work, she can be found either reading or spending time with her family.
Vincent Baluyot is a Licensed Dietitian in the Philippines and also a Nutritionist who has been working with doctors from various fields to provide comprehensive medical nutrition therapy services to every patient which includes checking a person’s nutritional status, giving the right foods or nutrients to treat conditions such as those caused by diabetes, heart disease, and kidney disease. His goal is to provide Kidney Nutrition Education to the persons who need it the most, to help improve their health through diet, and to improve their quality of life. As a care manager, he promotes being proactive in health care to allow patients to take control of their health before a problem starts or becomes too drastic.
Junelen Boguen is a licensed registered nurse. She always proves to be a reliable and solution oriented person. She is an excellent organizer who can quickly prioritize work based on clients’ needs. With her experience in a hospital setting and as a home health nurse, she learned to adapt, communicate, and can empathize well with patients.
Susan Fromme is an experienced and licensed Register Nurse dedicated to the mission of compassionate patient care. She has worked over 20 years as a care manager assisting patients in health maintenance, disease management and home safety. She has worked extensively with patients during times of transition and is very detail oriented, allowing families time to verbalize their concerns. She loves being a caring and compassionate nurse who is always looking to educate patients on the best way to help themselves with their health.
Amanda Kreisberg is a Medical Assistant and Phlebotomist registered through American Medical Technologists. She has been working with nephrology patients for 10 years, developing professional and empathetic relationships with them. Prior to becoming a clinical care professional, she also pursued a degree in psychology. She officially became a Chronic Care Manager in 2021 to continue to give patients the highest level of care possible. She is very detail oriented and patient care is her top priority. Her passion in life is to help patients reach their health goals and help to reduce any fears they may have related to their overall health. She continues to emphasize to all patients that she is always here for them to not only talk about their healthcare goals and health issues, but to help them in anyway with appointments, medications, and anything in between. She consistently strives to stay up to date with all things related to hypertension, nutrition, stress, and nephrology. When she is not working, she is a very proud dog mom to three dogs, and you can always find her reading a book.
Erica Castillo is a board certified Family Nurse Practitioner who has been specializing in Nephrology for 5 years. She was previously a registered nurse for 10 years, where she worked in an outpatient hemodialysis clinic, an inpatient medical surgical unit and a clinical decisions unit. She is currently working on her Doctorate in Nursing at Stony Brook University. While working as a dialysis nurse, she found that there was a lot more to learn in the field of Nephrology and was very interested in gaining more knowledge. She was given the opportunity to work with the physicians at PRINE, rounding at the hospitals and at outpatient dialysis facilities. She is now also part of the care managers team. She aims to help patients achieve their health care needs and goals whether it be in the inpatient or outpatient setting. She believes that building trust, showing compassion and keeping an individuals needs a priority is important in patient care. Providing constant education and follow up care can help patients achieve greater well being and most importantly having them feel that someone is willing to help and care for them. It is important that patients get the individual attention they need in order to help them achieve their goals, and she always tries to help them in any way possible. She enjoys following patients closely and having a relationship where they feel comfortable to discuss any questions or concerns. She prides herself on working and collaborating with a great group of care managers.
Lizbeth Rodriguez is a certified health coach and chronic care professional. She has a distinct knack for patient advocacy. She has always shown to make sure all of her patients are given the highest quality of care available. Care Management has enabled her to help patients navigate tough decisions that would otherwise be overwhelming. Lizbeth has extensive experience in coordinating patient care, with a specialization in renal transplant support. She demonstrates empathy for patients in need through excellent communication skills.
