Our team improves care coordination with Nephrologists and other doctors to keep patients on track with a unique care plan tailored to the individual needs of the kidney patient.

What You Can Expect In PCKC
Kidney Disease Education
PRINE has developed a curriculum of kidney education classes called PRINEd. The goal is to improve everyone's kidney health knowledge.
Engagement Building
The level of engagement corresponds with patient satisfaction and better outcomes. Patients in PCKC have care plans tailored to their specific needs based on engagement levels. We understand every patient is different, so we take an individualized approach to increasing engagement.
Depression Screening
Depression has a high prevalence (estimated at 22-40%) in Chronic Kidney Disease. The PRINE Care team actively screens all patients in our PCKC program and facilitates additional care or referrals when necessary.
Care Management Support
You will be assigned a PRINE Care Manager. Our goal is to empower you to actively participate in your care and keep you as healthy as possible. Your Care Manager will reach out regularly, coordinating and overseeing your care while working with you and your Nephrologist to achieve optimal kidney outcomes.

Our Interactive Kidney Disease Education classes are online or in person
Learn the basics of Chronic Kidney Disease (CKD). We will discuss common labs and explain their meaning. We explore the stages of CKD, general management principles, and expectations of progression.
In Choices 1 we review the different types of dialysis options available in-center and at home including hemodialysis and peritoneal dialysis.
In Choices 2 we discuss kidney transplantation and medical or palliative care in End Stage Kidney Disease.
Learn general strategies, tips, and tricks for proper nutrition for people living with Chronic Kidney Disease.
