PRINE Kidney Health Wellness, prescribed.
PRINEKidney Health

Your nephrologist visits four times a year.
We're with you the other 361 days.

Blood pressure control, nutrition, and lifestyle change are associated with slower CKD progression. That is the work we do with you, between every visit, and it is work insurance does not cover.

The Kidney Longevity Index
One number. Updated every quarter.
Built by your nephrologist.
The Kidney Longevity Index is a number between 0 and 100, updated every quarter. It is a proprietary score built by PRINE's nephrologists that combines your kidney lab values, your cardiometabolic control, your behavioral habits, and your physical function into a single number. It is not a diagnosis or a prognosis.

0 – 49

Action needed

One or more areas of your kidney health need attention. Your coach raises it with your care team and the next quarter is built around it. If a clinical visit is needed, that visit is billed to your insurance as it would be for any patient.

50 – 74

Stable

You are holding your ground. Your care team focuses coaching on the areas dragging the score until they move.

75 – 100

Optimized
The pieces the score tracks are in good shape. The work becomes holding what you have built.

The KLI is the foundation of the program. See how it works, why it exists, and how we are testing whether it holds up.

How the Kidney Longevity Index works →

The score draws from four areas of your health
Kidney Vitals

Your kidney lab values and the medications that protect kidney function

Cardiometabolic Control

Blood pressure, blood sugar, cholesterol, and weight are among the biggest modifiable drivers of progression

Behavioral Engagement

Medication adherence, nutrition, physical activity, sleep, and program participation

Functional Reserve

Strength and physical function, assessed by your care team at each quarterly visit

Kidney transplant recipients: Your score is called the Transplant Kidney Longevity Index (TKLI), a sibling score built specifically for the clinical reality of life after a kidney transplant. Same quarterly review, same format, adapted for your situation. Your care team will walk you through how it works.

Why we built this
For years, nephrologists like me have told patients the same thing: what happens between visits matters more than the visit itself. Then the visit concludes. We schedule a follow-up for three months, wish them well... and hope the plan holds.

That is not a failure of effort. It is a failure of structure. A nephrology visit is built to manage the numbers. It is not built to do the work behind them: the blood pressure at home, the food, the medications, the sleep, the strength. And the intangibles that never make it into a chart.

PRINE Kidney Health is the structure we built to do that work. A team that stays with you between visits, a score that shows you where to focus, and a plan that changes as you do. It does not replace your kidney care. It is the part that was always missing.

The gap

Your quarterly visit manages the number.
We manage what drives it.

Standard nephrology visits are quarterly. They review your labs and adjust your care plan. But kidney progression depends on what happens in the other 361 days: your blood pressure at home, your diet, your weight, your medications, and your sleep.

PRINE Kidney Health is the layer that lives in between. Structured behavioral and lifestyle support that insurance does not cover and that your quarterly visit does not have time for.

Your nephrology care stays exactly where it is. This program adds to it, never replaces it.

What PRINE already has in place
Board-certified nephrologists, an in-house lab, infusion centers, a renal dietitian, trained behavior-change coaches, and a kidney patient panel built over 25 years.
A board-certified nephrologist in the room, a physician-owned lab, infusion infrastructure already running, and a trust relationship built over decades. Med spas and direct-to-consumer platforms ignore kidney patients entirely. Hospital systems lack the agility.
New guidelines · June 2026
AHA · ACC · ADA · ASN

In June 2026, the AHA, ACC, ADA, and ASN released the first guideline for cardiovascular-kidney-metabolic syndrome

The guidelines formally recognize what PRINE's nephrologists have treated as clinical reality for decades: heart, kidney, and metabolic disease are deeply connected and must be managed together. They call for earlier screening, coordinated interdisciplinary care, and proactive behavioral intervention. The program is built around the same principles.
Why PRINE Kidney Health
Every nephrology platform of scale is paid by insurers.
None sells directly to patients.

A nephrologist in the room

Med spas and direct-to-consumer wellness platforms ignore kidney patients. They lack the clinical depth and accountability. Every protocol in this program is designed and reviewed by a board-certified nephrologist, because your kidneys are not a wellness lifestyle.

Honest about what it is

Insurance-covered care stays insurance-covered. This subscription delivers what insurance does not cover. No double billing. No implied treatment claims. No supplement-influencer language. The line is real and operationally enforced.

25 years of trust behind it

PRINE’s nephrologists have served kidney patients for more than 25 years.

A category of one

The first nephrology specialty practice in the country to offer a direct-to-patient kidney longevity program.

Designed for the long arc

Kidney health is not a 90-day program. The KLI tracks your trajectory across quarters and years. The goal is to be the team beside you for the second year, the fifth year, and the next decade.

For members who want to stay ahead of their numbers. More data, more access, all on the wellness side of the line.

Common questions

Things patients ask us.

No. Your nephrology care continues exactly as it is. This program sits alongside it and delivers what your quarterly nephrology visits do not have time for: behavioral coaching, lifestyle support, nutrition, and the KLI scorecard that tracks your progress between visits.

Yes. Insurance-billed services remain on the insurance side: nephrology visits, care management (CCM), remote monitoring (RPM), and nutrition therapy (MNT).

The Kidney Longevity Index is a number between 0 and 100, updated every quarter. It is a proprietary tool built by PRINE’s nephrologists that combines your kidney lab values, your cardiometabolic control, your behavioral habits, and your physical function into a single score. It is not a prognosis. It is a score that reflects how well the modifiable factors driving your kidney health are being managed right now, and whether they are improving over time.

Yes. Transplant recipients receive the Transplant Kidney Longevity Index (TKLI) a sibling score built specifically for the clinical reality of life after a kidney transplant. Same quarterly format, same care team structure, adapted for your situation. Your care team will walk you through how it differs from the standard KLI.

The program is currently offered to existing PRINE nephrology patients first. If you are not currently a PRINE patient, reach out and we will let you know when enrollment opens more broadly.

It is a fixed vitamin formulation offered as part of membership. Before each infusion, our team reviews your recent labs. If anything suggests it is not appropriate that day, we defer it. The formulation is not adjusted or customized in response to lab values.

It measures your lean mass, fat mass, and how it is distributed, as one input to your KLI. It is a wellness measurement, not a bone density or osteoporosis scan. If you need diagnostic bone testing, your physician orders that separately and it is billed to your insurance.

Resting metabolic rate measures how many calories your body burns at rest. We use it to tailor your nutrition and track your metabolic health over time. It is a wellness measurement and is not billed to insurance.

It is a set of advanced markers, including ApoB, lipoprotein(a), small dense LDL, and hs-CRP, included with the Premium membership as wellness measurements. It is not a substitute for the cardiac care your physician orders and bills. If a result suggests you need medical evaluation, your physician will discuss it with you and any resulting care is billed to your insurance as usual.

If you receive a kidney transplant, your membership continues on the transplant track (TKLI) at the same rate. If you start dialysis, you move to standard nephrology care, and any refund is handled under our membership policy.
