A single number, 0 to 100, built by PRINE nephrologists, that tracks the factors shaping your kidney longevity, quarter by quarter.
Kidney function declines in chronic kidney disease. How fast it declines varies widely from one person to the next, and that variation is well documented in the published literature. The gap it opens over time is the whole game.
Illustrative curves drawn from published ranges for eGFR decline in chronic kidney disease. Typical progression shown at 4 mL/min per year, slower progression at 1.5, both from a starting point of 55. Rates of decline vary widely between individuals; see KDIGO 2024 Clinical Practice Guideline for the Evaluation and Management of Chronic Kidney Disease. Not patient data, not a PRINE result, and not a projection of any individual outcome.
A sustained higher KLI is associated with a slower rate of decline and longer kidney survival than a sustained lower KLI, in people who started in the same place.
We are stating that plainly because it is a claim that can be proven wrong. The KLI is our attempt to measure, quarter by quarter, where a person sits in that range. It is not yet validated against long-term outcomes. We are testing it on our own members and we have published the timeline for that work below.
Construct validity. Does the score behave the way it should against measures already known to matter in chronic kidney disease? If it does not, the score is wrong and we change it.
Predictive validity. Do members who sustain a higher KLI™ show a slower rate of decline than members who do not, starting from the same place? This is the hypothesis above, and it is the test that can prove it wrong.
External validation. Review outside PRINE, on a population that is not ours. A score validated only by the people selling it is not validated.
Until that work is complete, the KLI™ is a wellness measure and a stated hypothesis. It is not a validated clinical instrument, and nothing on this page should be read as a prediction of any individual outcome.
So the goal is not a number that climbs.
It is a line that flattens.
Your KLI™ is a single score, 0 to 100, developed by PRINE nephrologists. It brings together four areas of your health into one number you can follow, reviewed with your care team every 90 days.
The labs and biomarkers that show how your kidneys are working.
The blood pressure and metabolic factors that most affect progression.
The daily habits and choices that shape your number.
The physical strength and resilience that protect independence.
KLI™, TKLI™, Kidney Longevity Index™ and Transplant Kidney Longevity Index™ are trademarks of PRINE Health. Scoring methodology is proprietary and confidential.
Everything your nephrologist does today carries on exactly as it does now, and it is billed to insurance exactly as it is now. The membership sits on top of that. It is the work between the visits, and the measurement that shows whether the work is landing.
Every other week, with someone who has your labs in front of them and knows what you are actually working on. Not a call center, and not a new person each time.
Working through what you actually eat, what to swap, and how to keep it kidney-safe on a normal week. The doing, not the handout.
Your KLI, a review of what moved and what did not, and a specific plan for the quarter ahead. Walked through with your care team, not emailed to you.
Frailty, balance, and physical reserve measured at every review rather than assumed, because they quietly limit everything else. Measured the same way each time, so the change is the signal.
Wellness services. Not covered by insurance, which is why there is a membership fee. Measurements taken in the program are wellness measurements, not diagnostic tests, and are not billed to insurance. If you qualify for covered medical nutrition therapy with a dietitian, that continues separately and is billed to your insurance, whether or not you are a member.
Billed to insurance, exactly as it is today. Nothing here moves behind the membership.
Every KLI™ falls into one of three zones. The zone tells you where you stand. The direction your kidney function is moving tells you how the work is going, and your care team reviews both. In any quarter where some of the underlying data is not available, your score may be calculated on partial data or paused, and your care team will tell you which.
A signal to focus. Your coach raises it with your care team and the next quarter is built around it.
A solid foundation. The work is holding gains and finding the next lever.
The pieces the score tracks are in good shape. The work becomes holding what you have built.
Transplant patient? Your score is the Transplant Kidney Longevity Index™ (TKLI™), our companion metric built for post-transplant care.
Ask About TKLIThe KLI™ was developed by practicing PRINE nephrologists out of the problem they see every week: two patients with the same numbers on paper, and completely different trajectories over the following five years. Nothing in existing practice put the things that explain that gap into one place a patient could see and act on.
It draws on the evidence base for slowing chronic kidney disease, which is large and is not ours. What is ours is the method for keeping all of it in view, quarter after quarter, and turning it into a single number a member can actually follow.
PRINE Health is one of the largest independent nephrology groups in the New York region. The KLI™ is used with our own patients before it is offered to anyone else.
Membership, pricing, and everything included live on the PRINE Kidney Health program page.
The program is open to adults with chronic kidney disease stages 2 through 4, and to kidney transplant recipients. If that is not you, we would still be glad to point you toward the right care.
The KLI™ is a wellness score, not a diagnosis or a clinical test, and it does not replace medical care. All decisions about your treatment remain with your treating clinician. Insurance-covered care stays with insurance. Your membership adds the wellness services insurance does not cover. KLI values and curves shown are illustrative and are not a projection of any individual result. Scoring methodology is proprietary to PRINE Kidney Health. PRINE Kidney Health is a program of PRINE Health Medical Group.
