Why the number matters

Two people can start in the same place
and be years apart.

Kidney function declines in chronic kidney disease. How fast it declines varies widely from one person to the next, and that variation is well documented in the published literature. The gap it opens over time is the whole game.

Our hypothesis

A sustained higher KLI is associated with a slower rate of decline and longer kidney survival than a sustained lower KLI, in people who started in the same place.

We are stating that plainly because it is a claim that can be proven wrong. The KLI is our attempt to measure, quarter by quarter, where a person sits in that range. It is not yet validated against long-term outcomes. We are testing it on our own members and we have published the timeline for that work below.

Validation timeline
Year one

Construct validity. Does the score behave the way it should against measures already known to matter in chronic kidney disease? If it does not, the score is wrong and we change it.

Year three

Predictive validity. Do members who sustain a higher KLI™ show a slower rate of decline than members who do not, starting from the same place? This is the hypothesis above, and it is the test that can prove it wrong.

Year three onward

External validation. Review outside PRINE, on a population that is not ours. A score validated only by the people selling it is not validated.

Until that work is complete, the KLI™ is a wellness measure and a stated hypothesis. It is not a validated clinical instrument, and nothing on this page should be read as a prediction of any individual outcome.

So the goal is not a number that climbs.
It is a line that flattens.

Everything your nephrologist does today carries on exactly as it does now, and it is billed to insurance exactly as it is now. The membership sits on top of that. It is the work between the visits, and the measurement that shows whether the work is landing.

Your KLI, a review of what moved and what did not, and a specific plan for the quarter ahead. Walked through with your care team, not emailed to you.

Strength and function, tested

Frailty, balance, and physical reserve measured at every review rather than assumed, because they quietly limit everything else. Measured the same way each time, so the change is the signal.

Wellness services. Not covered by insurance, which is why there is a membership fee. Measurements taken in the program are wellness measurements, not diagnostic tests, and are not billed to insurance. If you qualify for covered medical nutrition therapy with a dietitian, that continues separately and is billed to your insurance, whether or not you are a member.

Where your KLI sits, and where to take it next.

Every KLI™ falls into one of three zones. The zone tells you where you stand. The direction your kidney function is moving tells you how the work is going, and your care team reviews both. In any quarter where some of the underlying data is not available, your score may be calculated on partial data or paused, and your care team will tell you which.

Who built it

Built in a nephrology practice,
not in a software company.

The KLI™ was developed by practicing PRINE nephrologists out of the problem they see every week: two patients with the same numbers on paper, and completely different trajectories over the following five years. Nothing in existing practice put the things that explain that gap into one place a patient could see and act on.

It draws on the evidence base for slowing chronic kidney disease, which is large and is not ours. What is ours is the method for keeping all of it in view, quarter after quarter, and turning it into a single number a member can actually follow.

PRINE Health is one of the largest independent nephrology groups in the New York region. The KLI™ is used with our own patients before it is offered to anyone else.
